Catalyst Kidz forms and policies

                                                                                                       Please make known on your Kidz check in form any allergies
                                                                                                       your kids may have.  Also, please make staff aware if
                                                                                                       your children are currently ill (especially if they are running a
                                                                                                       fever over 100).  Our check out policy requires that you provide a
                                                                                                       number matching bracelet or label for children to be released to
                                                                                                       you or another guardian.  If you have any questions regarding our
                                                                                                       check-in/out and security policy, please feel free to ask any
                                                                                                       of our Catalyst Kidz Check-in team members.  

                                                                                                       We use a color coded system to designate our Catalyst Kidz 
                                                                                                       volunteers.
                                                                                                       Blue=Check In                Yellow=Infant/Toddler Area
                                                                                                       Orange=Preschool           Green=Elementary
